By submitting this form, I confirm and acknowledge that:

1. CPD Approval Credits prepay administrative review capacity only and do not guarantee CPD Program / Activity approval, which remains subject to independent assessment and applicable criteria.
2. CPD Approval Credits are governed by the IASE Africa CPD Accreditation Policy, which I have read and agree to be bound by.
3. CPD Approval Credits are non-transferable, non-refundable (except as expressly provided in policy), and subject to validity periods determined by IASE Africa.
4. The use and continued validity of CPD Approval Credits and any approved CPD programs depend on the maintenance of active CPD Provider Recognition.
5. Where this submission is made for quotation purposes, CPD Approval Credits are allocated only once payment is received against an issued quotation.
